You can provide consent orally, in writing, electronically or through an authorized representative. We will never sell any of your applicable medical information.

In the event of a change of control of our business, or substantially all of our business undertaking (inclusive of a merger or consolidation), or any other form of corporate reorganization, action or transfer between our corporate entities, you expressly consent us transferring your personal information to the new owner or successor entity to enable the continued performance of our services.
